Peroneal tendon surgery is a procedure which repairs or replaces part or all of the peroneal tendons. A tendon is a soft tissue that connects muscle to bone. There are two peroneal tendons which are located on the outside of the foot and ankle area (peroneus brevis and peroneus longus).
What is a peroneal tendon transfer?
The peroneus brevis works with the peroneus longus (PL) to turn the ankle and foot outward. The FHL to PB tendon transfer is a surgery to improve the function of the foot. The FHL is passed behind the ankle to the outside of the foot to either assist or replace the damaged PB tendon.
How long after peroneal tendon surgery can I walk?
After surgical repair of your peroneal tendon, you may need to be on crutches for about six weeks. Rest as much as possible with your ankle elevated above the level of your heart. Peroneal tendonitis and tendon ruptures can cause severe pain and immobility if left untreated, while early treatment can help patients avoid ruptures. But expect patients to require surgery when they have full ruptures or fail to respond to conservative treatment.

When a tendon is transferred and sewn into another position, the tendon transfer will need a period of time to heal, usually about one to two months. A splint or cast may be used, followed by therapy to teach you the new tendon function.

There are three requirements for tendon transfers to be successful: The muscle that the tendon is attached to has to be functional. It must contract at the proper time during walking. The soft tissue through which the tendon is to be transferred needs to be as normal as possible.
How do you sleep after peroneal tendon surgery?
You may sleep without your boot once you are 4 weeks from surgery. You should be non-weight bearing for the first 4 weeks after surgery. This means no walking on your ankle. You may, however, use your toes for balance.
How long do you wear a cast after tendon surgery?
Your tendon will slowly get stronger as you recover. You will need to wear a cast or a walking boot for 6 to 12 weeks after surgery. At first, it may be set to keep your foot pointed downward as the tendon heals.
What happens if you don’t repair a torn peroneal tendon?
If left untreated, eventually it can result in other foot and leg problems, such as inflammation and pain in the ligaments in the soles of your foot (plantar faciitis), tendinitis in other parts of your foot, shin splints, pain in your ankles, knees and hips and, in severe cases, arthritis in your foot.
